Services of Supply, [November] 26, 1918, I have examined Rumbold F.M. Col and find him physically qualified for transportation 1-22-[1919] Don P. Jones 1st Lt. M.C.

Description Form completed by 1st Lieutenant M.C. Don P. Jones stating that Colonel Frank M. Rumbold was qualified for transportation. Frank Meeker Rumbold served as Colonel of the 128th Field Artillery during World War I. He later served as Adjutant General of Missouri from 1925 to 1927.
