HEADQUARTERS EVACUATION HOSPITAL NO. 6 American Expiditionary Forces, France 15 November 1918. Special Order No. 83 1. In compliance with telegraphic instructions from HAEF., dated November 12, 1918., Operating Team No 158 headed by Major Ingraham, and composed of the following personnal, will proceed to Base Hospital No. 15, reporting upon arrival to the commanding Officer for duty. Major Clarence H. Ingraham Captain Thomas B. Holmes Dr. E.E. Leonard Pfc Joseph A. Jaffe Pfc Henry E. Thole The travel directed is necessary in the Military Service The Quartermaster Dept, will furnish commutation of rations for the period necessary to complete the journey, to the above mentioned soldiers and nurses.
