S.G.O.201-Leonard, Esther E., MCG. May 18, 1918. Dr. Esther E. Leonard, 6410A Michigan Avenue, St. Louis, Missouri. Madam: - I am directed by the Surgeon General to inclose herewith contract which should be dated and signed by you in the presence of a witness, whose signature should appear on the line provided therefor, and be returned to this office, in the inclosed franked envelope. No contract should be returned addressed to any person other than “The Surgeon General, U. S. Army.” One copy of the contract should be retained by you. You will note that the contract covers a period of one month. This does not imply that it will necessarily terminate at the end of that period. It can be continued indefinitely if such extension is mutually agreeable. Immediately upon signing this contract you should telegraph this office that you have done so, when steps will be taken to have you properly placed on duty. Do not enter upon your duties, nor perform any travel under your contract until you have received from The Adjutant General of the Army, either by telegraph or mail, an order to do so. Seven cents per mile is allowed a commutation for travel performed under proper orders, and upon the completion of any such travel, mileage voucher should be obtained from the nearest Quartermaster, which you should fill in, making no extension in the money column however, and then return the same together with two copies of the order directing the travel, to the Quartermaster for payment. At the end of each month during your service under your contract, you should obtain pay voucher from the nearest Quartermaster and return it properly prepared, together with your copy of contract, which will be returned to you by the Quartermaster after he has made notation of payment thereon. Just as soon as you have completed the duties for which your contract was made, and have returned to your home under proper orders, you should notify this office of that fact by telegraph, when your contract will be annulled. Description C.W. Ayars wrote this letter to Dr. Esther E. Leonard concerning the proper procedures for salary and travel reimbursements. Dr. Leonard served as a contract surgeon during World War I at the U.S. Army General Hospital No. 1 in New York City and at an evacuation hospital in Vichy, France.
