Dear Dr: Leonard: ... The Surgeon General directs me to forward the enclosed blanks which you will accomplish and present to the board of examiners for applicants for the Medical Reserve Corps at St. Louis, Missouri. The board will accomplish your physical and professional examination and forward the papers to this office, together with personal history sheet, two letters of recommendation and satisfactory evidence of your registration to practice medicine in the State where you reside. In the event that you are accepted you will be given a contract, as outlined in circular of information which is inclosed. Very truly yours, Bert W.
