[American Expeditionary Forces letterhead] 28 April 1919 To: The Surgeon General of the Army Washington, D.C., U.S.A. This is to certify that, so far as the present records of this Office show: 1. Captain Levi H. Fuson, M.C., U.S.A., has never been a Disbursing Officer of the Medical Department in the American E.F. (a) 2. He is not indebted to any hospital fund in the American E.F. for subsistence while a patient in hospital or on duty in hospital. (b) 3. He is not charged as a custodian of a Hospital Fund in the American E.F. (c) 4. He is not accountable for any property of the Medical Department. (d) NOTE: - (a) Proceeds from sale of a medical Property are sent direct to the financial Requisition Officer, American E. F. by the Officer making the sale, per Bulletin 45, Hq S.O.S., 1918, and do not pass through this office. (b) Bills against Officers for subsistence while patients in hospital or on duty in hospital in the American E.F. are being received covering past periods. (c) Hospital Funds in the American E. F. are being located of which this office has not previously had record. (d) Property vouchers and property returns are being received by this office covering past transactions. (e) If the receipt of any information outlined in (a). (b), (c), and (d) above, discloses any receipts, not accounted for, from sale of property, any indebtedness to Hospital Funds, Custodianship of Hospital Funds, or accountability for property, a supplemental statement of such facts will be forwarded to the Office of the Surgeon General, U.S.A. Description Lt. Col. Whitcomb issued this memorandum by order of the Chief surgeon to certify that Dr. Levi Fuson had never been a disbursing officer for or indebted to the Medical Department. Captain Fuson was in the U.S. Army Medical Corps. He served with Base Hospitals 21 and 210 in France. Base hospital 21, which was founded and staffed by Washington University in St. Louis, Missouri, was one of six that was selected for mobilization by the U.S. Government to support the British troops.
