CLARKE at Village of Saranac Lake, Franklin Co. N.Y., on August 30, 1920 state number 50924 required by law to be filed in the office of the State Department of Health; that the original thereof is on file in that office; that I have compared said copy with such original, and that it is a correct transcript therefrom and of the whole thereof.
